Position Summary:

We are seeking a dedicated and compassionate Chronic Care Coordinator to join our healthcare team. The Chronic Care Coordinator will be responsible for coordinating and managing the care of patients with chronic health conditions, ensuring they receive the support and resources necessary to manage their conditions effectively and improve their quality of life. 

Responsibilities:

Collaborate with healthcare providers, including physicians, nurses, and specialists, to develop comprehensive care plans for patients with chronic health conditions.

Conduct thorough assessments of patients’ medical history, current health status, and healthcare needs.

Educate patients and their families about their chronic health conditions, treatment options, medication management, and lifestyle modifications.

Monitor patients’ progress and adherence to treatment plans, and make adjustments as necessary to optimize health outcomes.

Coordinate healthcare services and referrals, including scheduling appointments, arranging transportation, and communicating with other healthcare providers.

Serve as a liaison between patients, healthcare providers, and community resources to ensure continuity of care and support services.

Document patient interactions, assessments, and interventions accurately and timely in electronic medical records (EMR) or other healthcare management systems.

Stay current on advancements in chronic disease management, evidence-based practices, and healthcare regulations.

Qualifications:

Certified Medical assistant (CMA)

Strong clinical assessment and critical thinking skills.

Excellent communication and interpersonal skills, with the ability to effectively engage and motivate patients.

Proficiency in electronic medical records (EMR) and other healthcare software applications.

Commitment to providing patient-centered care and advocating for the needs of individuals with chronic health conditions.

Ability to work collaboratively as part of a multidisciplinary healthcare team.

Bilingual proficiency in Hindi, Urdu, Arabic, Chinese and English preferred
