Community Health Worker Certificate
Lake Area Tech’s Community Health Worker certificate prepares you to support individuals as they navigate healthcare and social service systems. In one semester, you’ll build skills in client advocacy, care coordination, health education, and connecting people to the resources they need.
This fully online, full-time program is designed for individuals with at least one year of experience in a medical or social service setting. Students may begin in the fall or spring semester.
Upon completion, graduates are prepared for roles focused on community health support and outreach. Please note that this certificate is not eligible for financial aid, and a criminal background check is required to register as a certified Community Health Worker through the State of South Dakota.
Community Health Worker Certificate Learning Outcomes
The graduate of this certificate will be able to:
- Conduct comprehensive patient/client needs assessments and coordinate health system resources to connect clients with appropriate services while maintaining confidentiality.
- Apply evidence-based, culturally responsive strategies to deliver health education that promotes informed decision making and disease prevention.
- Coordinate community and healthcare resources to facilitate referrals and improve patient/client access to care within ethical guidelines.
