The Systems of Care coordinator will work with families, both in person and via telemed or telephonic to identify the family’s  individualized needs in the eight life domains.  The Systems of Care program is a wraparound approach to delivering services to at-risk youth and families, as identified by school systems and other community stakeholders.

The systems of care approach was developed as a way to better serve children and youth with serious mental health conditions, and their families. The goal is to provide children and their families with the services they need in their homes and communities in order to avoid the need for inpatient and residential treatment.

Qualifications:  The Systems of Care coordinator must have a minimum of a high school diploma or equivalent with preference for experience working with people, such as in a school setting or a case manager role.
