Position Summary
The Registered Nurse in Home Health provides skilled nursing care in accordance with a physician’s plan of treatment to clients at the housing with services (Hilltop Residence) or in the community, which can require travel to up to 25 miles (will receive reimbursement).

Duties / Responsibilities
- Initiates the plan of treatment and necessary revisions on an individual client basis.
- The RN assesses client’s physical, emotional, psychological and financial needs to the extent referrals are made to appropriate agency or professional.
- Performs skilled nursing services to the client in their residence.
- Evaluates and updates the physician and other caregivers on the client’s needs regularly.
- Initiates appropriate preventative and rehabilitative nursing procedures.
- Prepares clinical and progress notes on a timely basis.
- Counsels client and family in meeting nursing and related needs.
- Provides resources for the client and family to make decisions.
- Participates in in-services programs and provides in-services as requested to other staff persons.
- Maintains continuing education as required by state law.
- RN supervises effectively paraprofessionals assigned to the patient care coordinator’s clients.
- Maintains and fosters a good working relationship with clients and their families, staff, co-workers and community persons.
- Demonstrates and practices good safety and standard precautions.
- Will be assigned on-call hours one evening each week, and one weekend each month.
- Performs other related duties as assigned.
Education / Experience
- Must possess and maintain current Minnesota license as a Registered Nurse.
Knowledge / Skills / Abilities
- Must have a valid Minnesota Driver’s License.
- Must have a reliable transportation to travel to clients’ home, and proof of car insurance.
- Must possess the ability to effectively communicate both orally and in writing with clients, families, healthcare providers, co-workers and paraprofessional staff.
- Prioritizes and organizes work effectively and efficiently to accomplish workload in time allotted.
- Exceptional interpersonal skills that affords the ability to effectively interact with clients, family members, co-workers, leaders, providers, etc.
- Adaptable and flexible to changing situations.
- Able to deal with problems involving several variables in standardized situations.
