Position Description
Provides mental health support services to adults as part of a dynamic CARE team (Comprehensive Assistance
with Recovery and Empowerment). Client population served dependent on agency need with consideration for
clinician preference and expertise.
Responsibilities
• Serve adult clients with serious mental illnesses within an integrated system of care
• Apply evidenced-based interventions according to strengths and needs of the client
• Utilize culturally responsive approaches in working with diverse populations
• Complete comprehensive mental health assessments
• Develop, implement, and regularly review treatment plans
• Complete progress notes and other necessary documentation within Electronic Health Record (EHR)
according to agency, state, and federal guidelines
• Provide case management services
• Coordinate client care with other professionals and community partners
• Have knowledge of mental health diagnoses within the DSM-5-TR or its successor
• Respond to crisis situations compassionately and appropriately
• Deliver services in client homes, community, office settings, or wherever necessary to best serve clients
• Facilitate group sessions to assist clients in their mental health recovery through psychoeducation,
independent living skills, and prosocial activities
• Complete Medication Aide training to administer psychiatric medication, provide medication education,
and assist clients in their psychiatric stability
Requirements
• Minimum of an associate degree in the social sciences or human services field
• Knowledge of mental health services with willingness to further develop skills and understanding
• Adhere to HIPAA guidelines and agency policies
• Be self-directive while also seeking clinical supervision when appropriate
• Maintain calendars and manage workload effectively
• Demonstrate effective communication and organizational skills
• Establish and maintain effective working relationships with clients and colleagues
• Proficient in computer applications and related technology
• Meet deadlines
• Follow all local, state, and federal laws
• Adhere to agency code of ethics, policies, and procedures
• May need to use personal vehicle for work-related tasks when agency vehicles are unavailable
• Work hours may include some evening and weekend hours
• Must have a valid driver’s license
