Special Status Registration for CNA Students
** Starting March 2, 2020 you will be required to complete an INITIAL APPLICATION through the state of South Dakota.
Instructions for Registration If You Have a Current Employer:
1.) Complete FORM A (CNA Training Enrollment Verification Form) with your employer.

1a.) Applicant and Employer will sign FORM A BOTH SECTION A-2 & A-3 (CNA Training Enrollment Verification Form).
2.) Complete FORM B (Certified Nurse Aide (CNA) Initial Application)
2a.) Upload FORM A along with a completed FORM B and submit to the state.
3.) Wait to hear back from the State on Initial Application Approval.

4.) Once you have approval, complete FORM D below (CNA Registration To Lake Area Tech) below to enroll CNA training at Lake Area Tech.
5.) Lake Area Tech will reach out with account set up and log-in information.
Instructions for Registration If You Do Not Have A Current Employer or are enrolling for Dual Credit:
1.) Complete FORM A SECTION A-2 (CNA Training Enrollment Verification Form).

1a.) Complete FORM C (Background and Disciplinary Information) Below.
1b.) Sign FORM A SECTION A-2 (CNA Training Enrollment Verification Form) and attach to FORM C below.
1c.) Wait to receive signed FORM A SECTION A-3 back from Lake Area Tech via email, this could take up to 2 business days.
2.) Fill out FORM B (Certified Nurse Aide (CNA) Initial Application)
2a.) Upload FORM A when you submit Form B to the state.
3.) Wait to hear back from the State on Initial Application Approval.
